This notice, called a “Summary of Material Modifications” (“SMM”), is being provided to advise you of changes that the Board of Trustees have made to your Plan of Benefits under the UFCW Local 1500 Welfare Fund (“Fund’). After you have read this SMM, please keep it with your Summary Plan Description (“SPD”) so you will be reminded of this change.

12th Annual Bowl-A-Thon
On June 6, UFCW Local 1500 members, along with their families and friends came together for our 12th Annual Bowl-A-Thon. We shared an evening filled with fun, laughter, friendly competition, and great prizes. This event was a wonderful opportunity for our union family to connect outside of the workplace and enjoy a night together. Thank you to everyone who joined us and helped make this year’s Bowl-A-Thon another memorable success. We look forward to seeing you all again next year!
