Beginning July 1, UFCW Local 1500 members covered by the Anthem Full-Time, Special Part-Time, and ACA Part-Time Welfare Plans will transition their prescription drug coverage from Express Scripts/Evernorth to Prime Therapeutics.

This notice, called a “Summary of Material Modifications” (“SMM”), is being provided to advise you of changes that the Board of Trustees have made to your Plan of Benefits under the UFCW Local 1500 Welfare Fund (“Fund’). After you have read this SMM, please keep it with your Summary Plan Description (“SPD”) so you will be reminded of this change. 

On June 6, UFCW Local 1500 members, along with their families and friends came together for our 12th Annual Bowl-A-Thon. We shared an evening filled with fun, laughter, friendly competition, and great prizes.
